Complex case discharge delays, also known as bed blocking or bed delays, are a longstanding and growing challenge for hospitals across the nation. Legacy Health is experiencing these delays in our acute care facilities. At any given point in time there are 100+ patients who no longer require acute care but are unable to safely discharge. Legacy Meridian Park Medical Center stood up a transitional care unit to accommodate these patients until a safe discharge could be achieved. In 6 months, this unit has served over 100 patients and saved Legacy Health over 1000 acute care patient days. The unit has further reduced the cost of care for these patients (annualized $950K savings) and created a safe environment until safe discharge could be arranged.
